Pros: I only rated this a 5 on effectiveness because if someone wrapped a chain around your nose, wouldn't you quit what you were doing wrong? I won't use chain twitches.
Cons: Everything.

At certain times with some horses, twitches may be needed, but not a chain twitch. There are other kinds of twitches that aren't as painful, but do a good job. The "one man" twitch is a better alternative. I have actually seen people use a chain twitch and then for a long time, they couldn't do anything with horse's nose, mouth or head because the horse thought they was going to be hurt again. A homemade twitch out of a cotton lead-rope material works just as well and is not as painful and no risk of cutting into a horse's nose or scraping skin off of nose. I've heard of this happening with a chain twitch. If a person has to use this type of twitch to shoe their horse or when the vet comes out everytime, then they need to do a little more training with their horses.
