house breaking?????

We have had Olivia (Rio) since mid March and we are STILL working on the housebreaking. I would appreciate any suggestions. She is crated at night and when we are gone, and goes out immediately when we let her out. She does potty outside but also still potties inside if we are not constantly noticing her. Ideas???
