I have ordered many name tags through the years, many locally. The one I ordered for my Chelsea I ordered from a form that I got at my vet's office. Of all of the tags I had ever used, it lasted the longest ~ to the end of Chelsea's life (age 16), with her name and phone number still clearly visible. Chelsea's tag was in the shape of a bone and was stainless steel. Many others that I have ordered from other places (at pet shows, etc.) haven't lasted very long at all, and I've had to replace them. I really do recommend the tags that you can easily order from the form you can get at any vet's office.
