Tell the vet you want to try something else. If they say no, go to a different vet. Ivermectin is what we use at the shelter. The dogs are given it orally for over several months.
But really tell your vet you want a new treatment or get a different vet.

I had a mastiff puppy that was born with mange. It was passed on from mother to offspring. If you got the puppy from a breeder it is possible they are breeding a dog who is a carrier and should not be bred. My puppy was treated with injections of ivermectin, which did help but if she was stressed the mange would start to come back. I would discuss a treatment plan with your vet & I would also contact the owner of the mother to see if other puppies in the litter have any symptoms. I have seen whole litters of puppies born with mange because the mother was a carrier. I was a vet. tech. for over ten years and fortunately my puppies care was pretty much free because my employers gave their services free of charge and gave me the meds. at cost. Good luck hopefully it will clear up with treatment and your puppy will be happy & healthy. As for putting a collar on her to prevent the itching. How would you feel if you had an itch you could not scratch? pretty miserable I would imagine, keep the nails short and get something for the itching. Sometimes an anti-itch shampoo can help or something with oatmeal in it. You also should see if your vet can give you an eye ointment to put in your puppies eyes while dipping or bathing ( you can even put a little vaseline in and around the eye) it will prevent chemical burns to the eye if he splashes or you accidently get some by his eyes. The heat of the eye will dissolve the ointment or vaseline after awhile.

Your dog could be in need of a cortizone shot and bathing the dog can strip it of it's own natural oils to make the matter even worse.Definately make an appt. with the vet! They will also tell you the name of some oil to administer by pill form or mixing in food that will help the healing process.

Tell your vet that your dog is still itching like crazy and ask if you should try adding a new treatment. Trimming your dogs nails wouldn't hurt the situation either. Since your vet just recently saw your dog for this problem you can probably just call your vets office and leave a message with the receptionist about your concerns.
You can always get him a small hard plastic e-collar to prevent your dog from being able to scratch his head. Your vet will have a stronger version of an e-collar than what you can get at pet stores and ask your vet to recommend the right size. You can also call around to local vets and see who sells the hard e-collar cheapest.
I hope your dog starts feeling better soon. Poor puppy.
