Oh please do take her to the Vet immediately. Kidney failure is so common in senior cats and the cough could be cardiac related.
Go go go!
Pray she will be ok.

It is common for very senior cats to lose weight, but the suggestions here are on target. Hopefully, a vet check will not show anything like cancer or organ failure. Celebrate your cat's longevity and make these memories wonderful.

YOU NEED TO GET YOUR CAT TO THE VET. MY BOSS JUST PUT HER CAT, MAGIC, ASLEEP FROM SOMETHING SIMILAR TO THIS AND IT TURNED OUT TO BE CANCER

Considering the fact that you have an 18-year old cat, it may just be her old age catching up to. But, I would suggest to take her to the vet and see if she has an illness.
