The vet wants to give him a shot twice a month. I was wondering if anyone knows of a cream that I could use without having to give him a shot.

That depends where the infection is located. In the ear is the only place a dog would be able to have an ointment, anywhere else they'd lick it off. This is also only if it's not the inner-ear, that far in and the ointment won't reach.

There are pills available, but if your vet wants to do a shot, chances are it's a serious infection. You could try the pills first, but if they don't work, you're out the time and cost.

As Jillian said, creams are only good for outer ear infection. They don't do much for any other type of yeast infection. Injectable medications don't put as much stress on the liver as pills do and are work faster, so I would opt for the injections over the pills.
